HRIF Coordinator - High Risk Infant Follow up Clinic

SUMMARY: This position is accountable for the coordination of the High Risk Infant Follow-Up Program including the implementation and coordination of family-centered, developmentally supportive care for infants at risk and to ensure coordination and integration of services. Oversees clinical and operational functions, assuring a high quality of care, efficient use of resources and ongoing program improvements. Collaborates with families, the health care team and community resources in promoting coordination and continuity of care. Fosters staff and team growth and development, and provides clinical services utilizing family-centered, culturally sensitive practice.

REQUIREMENTS: This position requires: A) a Bachelor’s Degree; B) a license to practice in the State of California as an Occupational Therapist (OTR), Physical Therapist (PT), Psychologist (PSY), Registered Nurse (RN), or Social Worker (LCSW); C) a HeartCode Basic Life Support (BLS) certification within 30 days; and D) a minimum of 2 years experience in NICU, one of which should be in HRIF, discharge planning for NICU or similar community-based program.

POSITION DETAILS: Full time 80 hours per pay period, Day shift.
