Case Management Liaison - Case Management - PT

SUMMARY: The Case Management Liaison's (CML) primary responsibilities are to perform selected services and functions related to patient care coordination, utilization review, discharge planning and data collection for the Case Management Program under the direction of the Case Manager. This includes communicating with third party review organizations, ordering discharge equipment and services, arranging patient transportation and community services.

JOB REQUIREMENTS: High School Diploma or GED. Minimum 1 year experience in a health care environment. Prefer 2 years of college level courses.

POSITION DETAILS: Part-time, 40 hours per pay period

LOCATION: Madera, CA
