Case Manager Registered Nurse - Full Time

SUMMARY: This position is accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services available to meet a patient's health needs. Coordinates services provided to a specified group of patients, ensuring that the care is provided in an appropriate setting, and that the severity of illness and the intensity of service is consistent with the patient's clinical condition. Collaborates with the health care team members to ensure that the care provided meets desired patient outcomes with appropriate utilization of resources within expected time-frames across the continuum of care. This position practices within the scope of the Nursing Practice Act, and in accordance with the policies on Valley Children's Healthcare.

This position will primarily provide case management for the pediatric general medical unit, also known as Discovery, but may require cross training to other areas of specialty when coverage is needed.

EXPERIENCE:

• Minimum 2 years nursing experience
• Pediatric experience, hospital case management/utilization management experience desirable
• Bilingual skills desirable

EDUCATION/LICENSES/CERTIFICATIFICATIONS:

• Graduate of an accredited nursing program, Bachelor’s in nursing or a field that supports the case management process preferred
• Active California Registered Nurse licensure (RN)

POSITION DETAILS: Full Time, Hours are Mon-Fri, 8am -5pm. Weekend On-Call coverage is required on rotation.

PAY RANGE: $53.00 - $78.00
