Medical Social Worker - Adult Transition Clinic - Full Time

SUMMARY: Accountable for meeting the psychosocial needs of patients and families through clinical assessment, crisis intervention, brief therapy, case consultation with the healthcare team and collaboration with other professionals in the community.

REQUIREMENTS:

  • A Masters Degree from an accredited School of Social Work required.
  • Eligible to receive Licensed Clinical Social Worker (LCSW).
  • A minimum of six (6) months in a health care environment.
  • Bilingual skills preferred.

POSITION DETAILS: Full time, 80 hours per pay period.

PAY RANGE: $36.00-$56.00

LOCATION: Madera, CA
