Pharmacy Tech - Med Rec - Pharmacy - Per Diem

SUMMARY: This position is accountable for participating in medication reconciliation for newly admitted patients who have been identified as high risk for medication misinformation. Medication history will be obtained from the patient/caregiver and appropriate pharmacies. The technician enters or updates the medication list in the electronic health record. The technician will communicate completion of the medication reconciliation and identified discrepancies to the patient's care team (physician, nurse and pharmacist). The technician will also be responsible for participating in Quality Assurance/Quality Improvement projects related to medication reconciliation.

REQUIREMENTS: This position requires a High School Diploma or G.E.D. PHCH (CA Pharmacy Technician) and CPHT (Certified Pharmacy Technician) required. Minimum three (3) years hospital pharmacy, medical clinic, or retail pharmacy experience. Demonstrated keyboard and computer navigation skills required. Demonstrated knowledge of outpatient and retail drugs (prescription and non-prescription) and usual pediatric doses. Proficiency in Microsoft Suite.

POSITION DETAILS: Per Diem. Variable shifts, start times are currently 7:30, 10:00 and 12:30 days and weekends.
