Pharmacy Tech - Med Rec - Pharmacy - Per Diem

This position is accountable for participating in medication reconciliation and will obtain medication histories for newly admitted patients who have been identified as a higher risk for medication misinformation. Medication history will be obtained from the patient/patient's family and/or physician's office. The technician enters medication history into the electronic patient's database or updates the database if information already exists. The technician will communicate completion of the medication history to the patient's care team (physician, nurse and pharmacist).

The Inpatient Pharmacy Department at Valley Children’s utilizes evidence-based therapeutic guidelines to provide exceptional pharmaceutical care to our patients. Our highly trained pharmacists and pharmacy technicians work side-by-side with providers and other interprofessional teams to ensure optimal patient outcomes, while utilizing state-of-the-art technology to maximize patient safety. Inpatient Pharmacy provides clinical interdisciplinary pharmacy services in all inpatient areas including Rehab, PICU, NICU, Acute Care, and Oncology. We have numerous pharmacy advance practice protocols including antibiotic monitoring, anticoagulation, IV to oral conversion, and renal dosing. Other clinically supported services include the Emergency Department, Palliative Care, Investigational Drug oversight, and parenteral nutrition (including production). Our team members also support the following specialty clinics: Oncology, Hematology, Rheumatology, Pulmonology/CF, Endocrinology, Neurology, Cardiology and our complex primary care center. Many pharmacists hold collaborative practice agreements to optimize medication therapy management and collaborate to enhance patient care.

EXPERIENCE:

• Minimum three (3) years Hospital pharmacy, medical clinic, or retail pharmacy experience required
• Proficiency with the Microsoft Office Suite
• Demonstrated knowledge of outpatient and retail drugs (prescription and non-prescription) and usual pediatric doses

EDUCATION/LICENSES/CERTIFICATIONS:

• High School diploma as accredited by the US Department of Education or GED required
• PTCH - California Pharmacy Technician license required
• CPHT - Certified Pharmacy Technician Board preferred
